[OpenAFS] 3rd FileServer started salvaging inexplicably

Running openafs 1.2.8 on redhat 7.3

For the third time this week, an afs fs just experienced the infamous 
bug where the fileserver starts salvaging but the fileserver processes 
aren't dead (so, fs then attempts to restart, fails, salvages, attempts 
to restart, fails, salvages, etc).

In the case of this file server and another file server which did this 
yesterday, the last thing in FileLog before the fileserver inexplicably 
started salvaging is:

CB: new identity for host ip:port, deleting

Looks like this time (unlike when the other two fileservers experienced 
this issue on Sunday and on Monday), a core was also dumped. We can 
certainly send that along to the appropriate person(s) for perusal.
