    windows: native versions of ih_pread and ih_pwrite
    
    Separate the windows code out in ihandle.c to reduce dependency on ntops.
    
    As an aid to future threading issues, pass the offset in an OVERLAP
    rather than doing a separate SetFilePointerEx.
