[OpenAFS] Corrupted Volume

Hello,
one of my user volumes died last week(needed salvage, with lots of
orphaned files) and I got the following from the Filelog about that
time:
Thu Feb 28 02:28:03 2002 CopyOnWrite failed: volume 536870939 in
partition /vice
pb (tried reading 8192, read 0, wrote 0, errno 4) volume needs salvage

029.vol; volume needs salvage; error=101
536870939 is the volume id for the volume with the corruption.
this is a redhat 7.2 afs server running openafs 1.2.2. kernel version
2.4.9-13 this is a uniprocessor i686 box
this has happened several other times with this same user, and at least
once for a couple of others.
any suggestions what might be causing this?
