[OpenAFS] fileserver hang #4

We had another fileserver hang.  The fourth one in as many weeks.  The
volume size that was not responding is about 6G.  rxdebug showed 143
calls waiting for a thread.  This volume serves images to a web server
through a series of Perl cgi scripts.  I see nothing in the logs to
explain this.  Openafs 1.2.11, RedHat AS 3.0.

Kill -9 followed by bos start was successful and the salvager ran
normally.
