[OpenAFS] missing mount point RO volumes

We were getting a time out from our web servers when they tried to access
files in our AFS cell. It turned out that the mount point for this
directory was gone at the two RO sites. I looked at the RW volume and it
needed a vos release. That cured the mount point problem and the web
servers resumed normal operation.
My questions:
What could have caused the RO volumes to become unmounted?
If the RO volumes are inaccessible, doesn't the RW volume become the
failover site?
What can I do to prevent this in the future?
