[OpenAFS] making proper debug report

I was just looking on the release notes for the current version of
openafs on windows and the link on 'How to debug problems' doesn't
work... and there is no section 4...

sorry to bug becuase I know it has been posted here multiple times...
but were are the instructions on making proper debug report?
