[OpenAFS-port-darwin] Contextual Menu Item for AFS Premissions

I do not have permission from Stanford to make a public release of the 
source code.

However I have been given permission to pass on the original source with 
my modifications to the official OpenAFS maintainers if they want to add 
this to the distribution and take over maintenance.  My contact at 
Stanford has been Jim Brown.
