    doc: Document current coding style

The current code-style-guide does not document or is inconsistent with
    the current styles being used.

    - Explicitly state the rules for tabs
      - Clarify for which files tabs must be used (C and C-like files)
    - Document how preprocessor directives are indented
    - Recommend commenting an #endif with the condition it closes when the
matching #if is not easy to see
    - Correct the prose for the indent utility
      - List the indent options together
    - Use a visual marker in code examples to illustrate tabs
      - Remove all physical tabs from the file

Add a statement that indent does not preserve the code style for
    preprocessor indentation.
