[OpenAFS] OpenSSH and AFS Token Passing

And of course, I attach the wrong patch.  The version I meant to attach
surrounds the #include <sys/ioccom.h> with an #ifndef/#endif.  It's only
a minor cosmetic/style error, but you can find the correct version at:
