[OpenAFS] salvager failure to repair root directory in volume

FYI:

I recently had a disaster with root directory corruption in
a volume.  I had to resort to the source code and repair
it by hand with emacs.

This was perhaps caused by a power failure to the linux
machine involved, and the fact that the volume was on
a non-journalled filesystem (ext2).  The various data
structures in the file involved were expecting an
extra page at the end of the file that did not exist.

The salvager could use some work is I guess what I am
passing along.
