I have run into problems with the salvager being unable to salvage
properly.  In my case, the data were precious, so I ended up getting
out the source code and learning about the on-disk format so I could
do the salvaging myself.

In an AFS variant called MRAFS, there is an option for the salvager
to rebuild directories.  I don't have access to it, but wonder whether it
would help in such situations -- the current salvager obviously has holes.
