Embarassingly, it looks like no one here can find the source code.  :/
(In our defense, we did this many years ago and we're trying to retire the
entire service, but still.  Gah.)

We didn't do anything particularly complicated, though, and we didn't use
PAM.  We just took proftpd from a few years back, wormed our way into the
area where it did password authentication, and replaced that code with
code to do a setpag, a Kerberos v4 authentication, and acquire AFS tokens.
PAM is a much better solution.
