[OpenAFS] fileserver callback tieup

I've had two different fileservers rendered unusable recently.  On each
machine a file has been held open by a client, or the process the client
is running, causing the callback table to backlog on the fileserver. 
The fileserver then becomes increasingly unable to answer requests.  A
'bos salvage' or  fs shutdown has no effect due to this hung file.  The
only way to regain activity is to kill -9 the fs process, or reboot the
machine.

I'm trying to determine if it's an older client gone awry, or if it is a
server tuning issue.  Your thoughts are welcome on this, or any other
possible cause.  Thanks.

Server platform is RHAS 3 - OpenAFS 1.2.11
Current tuning flags are: fileserver -L -p 24 -l 1000 -s 1000 -b 200 -cb
65535 -vc 1000 -rxpck 2000 -udpsize 1048576
