Surely that's three directories per user?

How does that become a million files?

Courier-IMAP is used by huge sites with hundreds of thousands of users
(who surely generate hundreds of million of files altogether)... perhaps
you should look at how they deal with this?

IMAP server comparisons is outside the scope of this list.

The point is, this looks very much like a Courier issue; you can't expect
good results from any filesystem with a million files in a single
directory.
