[OpenAFS] mountall: CANNOT READ: BLK 0

Folks,
I've just compiled openafs version 1.2.11 for Solaris 9 v.112233-11.  
Following the boilerplate installation instructions, I'm having a 
problem mounting /vicepa.  When I do mountall I get:

[root@rafs12 afs]$mountall
checking ufs filesystems
----Open AFS (R) openafs 1.2.11 fsck----
/dev/rdsk/c1t1d0s7: CANNOT READ: BLK 0
/dev/rdsk/c1t1d0s7: UNEXPECTED INCONSISTENCY; RUN fsck MANUALLY.

I must of omitted something.

I've:
1. newfs the partition (/dev/rdsk/c1t1d0s7)
2. Added this to vfstab: /dev/dsk/c1t1d0s7       /dev/rdsk/c1t1d0s7      
/vicepa afs     2       yes     nologging

I also received these messages in Salvager.log
